Clinical Observation of 168 Cases of Chest Injury

Abstract
Chest cavity contains vital organs such as respiratory and main cardiovascular systems, so chest trauma requires emergent adequate management.
This report consists of 168 cases of chest injury treated at the General Surgery Service of Capital Army Hospital during these 8 years from Aug. 1960 to July 1968.
1) 168 cases consists of 118 cases (70%) of penetrating wound and 50 cases(30%) of non-penetrating wound. Of them, 78 cases (46.8%) were by gun shot, 27(17.4%) by shrapnell, 42 (25%) by auto-acciedent and 11 (6.6%) by other causes.
2) Three fourth of 168 cases (78.6%) were managed within 6 hours after trauma.
3) 105 cases (62.5%) were treated with closed thoracotomy, 11 (6.5%) with open thoracotomy, 15 (8.9%) with repeated thoracentesis and 29 (17.2%) with conservative therapy. Tracheostomy was performed for 8 cases (4.9%)
4) Complications were noted in 38 cases (22.6%). The most common complication was 13 cases(34.2%) of atelectasis and the next was 8 cases (21.1%) of wound infection.
5) The expired cases were 16 cases (9.4%). They had received severe multiple injuries. The most common cause of death was acute cardiac arrest and acute cardiopulmonary insufficiency (8 cases; 50%). The other causes were acute renal insufficiency (3 cases; 18.7%), shock (2 cases; 12.5%), peritonitis (1 case; 6.3%) and mediastinitis (1 cases; 6.3%).
